IM4Q HABILITATION SURVEY

 

The Office of Developmental Programs has a Habilitation Survey that obtains information on:

 

bulletSatisfaction and outcomes of adults age 18 and over, and their families, with adult day training, vocational training and employment services

 

bulletCharacteristics of adult non-residential services from the perspective of adult training, vocational and employment support agencies

The survey and design implementation have been developed by the Office of Developmental Programs in collaboration with the Institute on Disabilities at Temple University, the IM4Q Steering Committee, PARF, and a stakeholder workgroup.

This survey has been developed to obtain information on characteristics, satisfaction and outcomes related to non-residential services provided in the Armstrong & Indiana Counties MH/MR Program. our current data on the adult non-residential service system is sparse, and additional data is needed as part of the Office of Developmental Programs planning effort to promote employment and other self-determination outcomes for adults.

The Arc of Indiana county has been asked to conduct the Habilitation Surveys in Armstrong and Indiana Counties.
